Bases loaded no one out. Runners running on the pitch. Ball hit to the Center Fielder. One out. Throw the ball to 2nd base. Two out. Throw the ball to 1st base. 3 out. The runner on 3rd base already scored so the run counts for now. Appeal to 3rd base for leaving too soon. Out. Quadruple Play! In real life Brooks Robinson hit into a Triple Play while bringing home a run. The bases were loaded no one out. He hit into the normal Double Play with the runner on 3rd scoring. But the runner on 2nd base tried to score and was out at Home plate for a Triple Play. There was a play I read about that happened before 1920 with the bases loaded and no one out. The play went Home to 3rd to 2nd to 1st for what seemed to be a Quadruple Play. But the Umpire ruled the inning was over after the 3rd out at 2nd base.

Interesting thought process here barkeep. At least the umpire was smart enough to understand three outs.
